At about 4:00 a.m. on June 26, 2020, an employee was setting up a newly installed ultrasonic welder. While he was adjusting the head of the equipment on the mount relative to the welding plate, a gap was created between the head of the equipment and the welding fixture. When the computer recognized that the machine was too far out of adjustment, the welding fixture recoiled back up, closing the gap on the employee's left index finger. The finger was partially amputated.

An employee was operating a machine that prints logos onto fabric materials when the machine began to leak ink. As the employee was attempting to remove ink from the roller using a towel, the rollers caught the towel and pulled her hand into the machine. Her right little finger was crushed and de-gloved. Her finger had to be surgically amputated.

On November 13, 2015, at approximately 2:30 PM, an employee was working from an aerial lift and signaling a crane lifting a pipe. The crane was overloaded, tipped, and struck the aerial basket the employee was riding in. The employee was bounced out of the aerial lift. When the crane sat back down, the employee was bounced back in the aerial lift and back out again. The employee suffered a spinal fracture, broken ribs, and multiple facial fractures, requiring hospitalization.

On July 17, 2015, at approximately 1:45pm, an employee was hospitalized after suffering two broken bones to his lower left leg. Before the incident, the employee was standing on the third rung of a 12-foot stepladder, tightening bolts on a flange with a torque wrench. While applying pressure to the wrench, a reducer on the wrench broke, causing the employee to lose his balance. As he stepped down 34" to the gravel surface, he landed awkwardly on his left foot, causing fractures.
