On September 6, 2016, at approximately 6:15 p.m., an employee was entering the chicken house to catch chickens. While walking into the chicken house, an overhead bi-fold door, approximately 10' wide by 10' tall, fell and struck the employee on the head, knocking him to the floor. Threads of a nylon strap that held the door in the upright position broke and allowed the 150-pound door to fall. The door continued falling onto his left foot, fracturing his left ankle. He was hospitalized for surgery
