An employee was inside an aerial lift basket approximately 140 feet above ground level prepping for installation of an intake hood. The employee was connecting a welding lead to the welding machine. An overhead bridge crane struck the aerial lift. The boom of the aerial lift collapsed and the employee was catapulted out of the basket falling 34 feet onto the top section of the crane. An employee was standing on steel floor plates. The floor plates shifted under the weight of a forklift as it backed over them. The employee lost balance; his left foot slid down and became trapped between floor plates with the weight of the forklift pushing on the plates. The employee sustained the amputation of two toes.

An employee was adjusting the high/low switch on his truck-mounted welder. He was turning around when he lost his footing because it had just rained and his truck bed was slick. He went to steady himself and reached to grab onto something with his hands. His right hand slid underneath the cowling of his welder and two fingers were severely lacerated by the motor fan blades. He was hospitalized.
