On October 30, 2015, at approximately 8:30am, an employee was hospitalized after being struck by a 14 inch diameter polyethylene pipe. The employee was raising a hydraulic jack on the McElroy 618 machine when the pipe under pressure released from the cradle and struck the employee in the face and chest. The employee's injuries were bruising of the chest, 12 to 13 facial fractures, loosened teeth, and a broken nose. The employee was hospitalized.
