An employee was feeding a piece of 1/4"-thick by 2"-wide by 2'-long steel flat bar into an electric plate rolling machine. The employee noticed that the material was feeding crooked. When he tried to straighten it, his right hand was pulled into the rolling mechanism, crushing the index and middle fingers between the rollers. He used his left hand to push the roll-release lever, which freed his right hand from the machine.

On 12/16/15, an employee's hand was caught-in the roller of a dough rolling machine while the employee was attempting to clean the machine with a rag. The rag was caught-in the moving roller and pulled the employee's hand into the machine. The employee was admitted to the hospital with a severe hand injury and is awaiting surgery.
