A sensor sent the jaw block (hydraulic-operated) down and amputated the employee's left-hand ring finger (half of finger) and pinky (1st joint of finger).

An employee was operating the 1708 machine, finishing a tube. The machine cycled then stopped. The employee noticed the machine had stopped and removed the guarding to investigate the problem. The employee saw that the material had pushed the home sensor and then pushed the material back manually, causing it to move forward again. The material got stuck and at the same time pushed the employee's left hand into the clamp, causing an amputation of the left pinky finger, which left bone exposed.
