An employee was cutting a piece of steel with a torch. About 40 feet away, an excavator with a grapple attachment was pulling apart two other pieces of steel that were connected with bolts. When those two pieces were pulled apart, the bolts broke and one of the pieces flew out the side of the grapple. It struck the employee in the hand and right leg, causing hand fractures and lacerations and a broken femur. The employee was hospitalized.

On July 21, 2015, at approximately 8:30 p.m., an employee was sweeping up sawdust and carrying a paint can at the ECG Crankshaft mill in Sharon, Pennsylvania. Conductors were sticking out of the metal conduit beneath a saw table at the time. The employee reached down to pick up the paint can, which then contacted the live conduit. The can exploded, burning the employee's right forearm. The employee was hospitalized overnight.
