On September 1, 2015, an employee parked his semi truck in the shop on the cement floor for servicing. He was going to lift the truck onto jack stands by first lifting it with the jack lift. While lifting the truck, the employee got down to the floor level to look beneath it. The handle of the jack lift fell, striking his left-hand little finger against the floor and resulting in a fingertip amputation.
