On 05/11/2015, at approximately 12:55 a.m., an employee was loading a bin in the loadout area. He was monitoring the material being filled into the bag of litter pellets when the loader dropped some product on the floor. He moved out of the way, stepped on some pellets, slipped, and fell down on the concrete floor. He got up and walked around, but later felt some pain and was hospitalized.
