Two employees had lifted a 200-pound hatch and attached a gas shock to the hatch assembly (to be prototyped as a hold-open device). A third employee reached up to pull on the hatch and determine the force needed for the hatch to close with the gas shock in place. The tack welds that held one end of the gas shock broke, causing the hatch to close on the employee's right hand.
