An employee was on a roller conveyor attempting to replace a chain on a sprocket to get a pipe to move. Another pipe was placed on the conveyor, causing the sprocket to move. Both of the employee's hands were then caught in the sprocket, amputating the left index finger at the first knuckle and the left middle fingertip.

A 9-inch joint of pipe was stuck crossways on top of the kicker and the conveyor system between the pipe threading machine and the coupling area. The injured employee was assisting another employee in clearing the jam and was struck by the pneumatic kicker that kicks the pipe up onto the conveyor. The employee suffered multiple rib fractures on the left side and a possible hernia.

An employee was inserting a 5" pipe into a large section of duct work. He needed a come-a-long that was at the other end of the pipe. He climbed onto the pipe and started walking on the pipe to reach the come-a-long. When he reach a flange, he stepped over it and then fell approximately 6' to the floor. The employee sustained bruised ribs, a bulging disc and a grade two kidney tear.
