An employee was cutting lumber with a pull saw. He had accumulated some scrap lumber underneath the blade. He turned the saw off and proceeded to clean under the saw blade and guard when his hand hit the blade, lacerating his left thumb and index finger. The blade had not come to a complete stop at the time of the incident.

The injured employee was positioning wooden chock blocks/boards for leveling a piece of equipment. Another employee tossed a board towards the board the injured employee was holding. It struck the board the injured employee was holding and smashed the tip of the employee's right ring finger. The employee suffered an open fracture and avulsion and underwent a medical amputation of about 1/2 inch off the tip of the finger.

An employee was standing on a piece of equipment to assist in a maintenance task when a second employee inadvertently started the equipment. The injured employees' left foot then became caught and fractured in a rotating piece of machinery. The equipment was unguarded at the time.

An employee putting thread protectors on drill pipe was resting his hand on a TM-80 iron rough neck. The rough neck operator did not see his hand and started retracting the equipment, which caught the employee's right hand in a pinch point and broke a bone.

An employee exited his dump truck and approached a trackhoe operator. The trackhoe had a piece of concrete barrier in its grasp about 6 inches off the ground. The concrete barrier rolled onto the employee's right foot and leg, resulting in broken leg/foot.
