An employee was moving a cart filled with cardboard to a dumpster. As he lifted the cart, it slid and fell into the dumpster. When he pulled his hand away, his left hand was caught on the cart front pivot bar and the side of the dumpster, de-gloving his middle fingertip.

An employee saw a pallet fall off a forklift into a bin where it did not belong. He left his forklift and went over to the other forklift. He rode the forks of the forklift to a 6- to 8-foot high elevation and retrieved the pallet. As he was riding the forks down, his right-hand ring fingertip became trapped and was amputated between the mast and the rack of the forklift.
