At about 2:30 p.m. on November 16, 2021, an employee was checking the sight glass for a liquid level on an ammonia refrigeration system. The sight glass broke, and liquid ammonia sprayed out of the opening onto the employee's face, chest, and throat area, resulting in chemical burns.

On 09/24/2021, at approximately 4:30 PM, three employees were cleaning a toffee cooker. The toffee cooker consists of an upper plate and a lower plate that are raised and lowered to separate the cooker. The toffee cooker was locked out/tagged out. An employee was using a table saw to cut some weed wacker cord when the cord became tangled and pulled the employee's left-hand fingers into the blade. Two of the employee's fingers were completely amputated, and one was partially amputated.
