An employee was operating a chip spreader machine down a hill. The spreader began to skid when he applied the brake. The wheels locked and the spreader continued to slide down the hill. The spreader impacted a ditch and spun 180 degrees. The operator was thrown off the machine and landed on the bank of the ditch causing injury to his lower back.

An employee was training a new employee on how to operate a hostler tractor to hook into a trailer. The trainee high-hooked the trailer and pinned the employee's left leg between the trailer and the tractor. The employee suffered a shattered left ankle.
