An employee was loading a part onto a slotter when the part began to fall off of the holding fixture. While straightening the part out on the fixture, the employee actuated the slotter with her other hand. Her right index fingertip was then caught and amputated between the vice fixture and the part.

An employee was troubleshooting a press. The powdered metal is gravity fed through a tube into the die and the die shoe was not moving properly. The press was put into set up mode and the employee attempted to jog the shoe slightly. While the arm was jogging, a small metal arm of the shoe caught the employee's finger and pinched it between the shaker and the shake stop box. The fingertip was later amputated.

An employee was placing cardboard into the cardboard compactor chute while standing on a work platform with railings. The employee stepped through the railings to step on the cardboard and push it down further into the chute. The compactor was engaged at the time. The employee's leg got caught and became entangled in the moving cardboard. The employee suffered a broken leg and required hospitalization for surgery. The leg had to be amputated at the knee.
