The first floor cable line was still connected and under tension, which destabilized the ladder during his descent. Feeling the ladder shifting under him, he dismounted from the fourth or fifth rung up and landed on his side, fracturing his left wrist. He was hospitalized and had surgery.

His hand then contacted the coil and was crushed between it and the hold-down roller. His right index finger had to he surgically amputated. The machine was unguarded at the time of the incident.

On or about December 26, 2015, at 12:40 pm and employee was pushing a two-yard trailer can dumpster up to the back of the garbage truck when the front caster wheel of the dumpster seized up and twisted sideways pitching the dumpster forward and pinching the tip of the his right ring finger between the dumpster's trunnion bar and the truck. He was transported to the hospital where the tip of his right ring finger was amputated and repaired.
