An employee was checking the coolant level in a radiator for one of the rig's generators. When he twisted the radiator cap, the cap came off and he was sprayed with hot coolant resulting in burns to his right arm, shoulder, chest and scapula. Immediately afterward he ran towards the rig manager's trailer and while doing so tripped and fell resulting in a broken right clavicle and cracked scapula.

An employee was on the back of a goose neck flat bed trailer approximately four feet high unloading racks made out of pipe. As he started to unbind chains on two racks they slipped and started to fall. The employee then fell off the trailer to the ground and was pinned between two racks. The employee suffered an abdominal injury as a result of the incident.

An employee was disconnecting a piece of flow iron from a pump. He was supporting the piece of pipe with his left hand and hitting it with his right hand when it suddenly disconnected. Due to the weight of the pipe, it dropped about 1 foot, catching his left ring finger between the iron and rear bumper of the pump and amputating the fingertip. He required surgery.

On or about February 12, 2015, a worker was trying to light the heater treater In The Barriers Well, but the gas valve was not fully shut causing the gas to flash in his face. The Well was shut in and the worker was trying to get it back online. The worker sustained injuries to his face and right hand.
