An employee was disconnecting a braided metal flex hose that was supplying air to a furnace for the de-coking process. A supplier sent an alternate feed to the furnace, which required the furnace to remain online. The employee went to disconnect the flex hose from the furnace and received second degree burns to the face and first degree burns to the torso from steam/hot condensate. The line was not depressurized at the time of the incident.

An employee was helping to inspect an electrical line and repairing a jumper that came off the transformer switch. The employee pulled two of the three switches to de-energize the pole line and then repositioned his bucket truck to get to the next switch. When he grabbed the wire to reinstall it onto the third switch, the switch was energized and caused third degree burns to his index, middle, and ring fingers on both hands.

An employee was taking off the access panel of the E Line Coagulation Tank to clean the excess solids out of it. As the employee was doing that the residuals (adjusted pH hot water 150-160 degrees Fahrenheit, liquid latex, and 93% sulfuric acid) splashed onto the employee. The pH of the residuals was approximately 3.2. The employee received second degree burns down the right side of his body. The employee was hospitalized.
