An employee was driving a forklift into a trailer. Before the employee could get completely in the trailer, the truck pulled forward, causing the forklift to fall sideways between the back of the trailer and the loading dock plate. The truck then pulled forward some more, dragging the employee and the forklift. The forklift then fell to the ground on top of the employee, who suffered a fractured pelvis and fractured vertebrae.
