An employee was performing maintenance on a pump at a gas well frac job. The employee removed the top cap off of the pump and removed the top valve. The employee went to remove the bottom valve and the pump stroked, catching and crushing the employee's right hand. The employee suffered a hand injury and was hospitalized. | |||||

On June 1, 2017, at approximately 4:00 a.m., an employee was about to perform maintenance on an oil and gas down-hole mechanical setting tool. The employee picked up the 70 lb. setting tool to bump it on the ground and release the pressure. The tool was slippery from being covered with honey oil, and the employee's grip slipped. His right middle finger was then crushed and amputated by the setting tool. | |||||

On October 10, 2016, at approximately 4:30 p.m., an employee was helping to remove a light plant generator from an excavation. A 9-foot chain was attached to telehandler forks and the light plant was lifted from the excavation and transported to a flatbed pickup truck located below a power line. While standing on the ground, the employee used his hand to guide the light plant onto the 4' high flatbed. The telehandler forks made contact with the overhead 7,200-volt line. On March 28, 2016, at approximately 11:20 p.m., an employee exited his excavator and peered into the bin where he was placing additives. Wearing metatarsal gloves, the employee placed his left hand on the side of the bin. A piece of metal, used as a trough, was not secure and slid against his hand, amputating his left pinky finger down to the knuckle. |
