On August 23, 2020, an employee was checking the pressure on compressed oxygen cylinders used for boiler chemistry control. When the employee opened a cylinder manifold valve, the stainless steel braided hose connecting the cylinder cradle to the pressure gauge failed and whipped around, striking the employee on the back and the left elbow. On May 6, 2020, at approximately 7:45 p.m., an employee stopped a lathe and pulled a nylon strap through the center of a 13-inch long, 8-inch diameter part. The nylon strap was attached to a jib crane, and the employee needed to jog the lathe to lock the chuck. The lathe did not jog and instead began running at operational speed. He had his left hand on the winch line to help with the slack; when he turned, his left little finger was drawn into the pinch point created by the winch line and spool.
