At 12:02 p.m. on August 26, 2021, an employee lost consciousness while working on a rooftop. The employee slid off the rooftop, fell about 30 feet, and suffered fractures to a shoulder blade, the pelvis, three ribs, and both heels. The employee was hospitalized. Fall protection was not being worn at the time of the incident.

An employee was troubleshooting a stamper machine. The part-sensing proximity switch was malfunctioning. The employee reached through the part access opening in the guard to adjust the proximity switch bracket. The machine cycled, causing the employee's left middle finger to be pulled between the arbor and the embossing stamper. The finger was amputated between the first and second knuckles.

An employee was attempting to drain a 4-foot wide and 4-foot tall tank that had an open lid that was missing a counterweight. The tank was being moved into place by a forklift. When the employee instructed the forklift operator to lift the tank, the lift caused the lid to close. The employee's left hand was positioned near the opening, and the closing lid smashed the third and fourth fingertips of the employee's left hand. The fingertips had to be surgically amputated.

In the shelf line area, an employee was reaching over a conveyor belt to inspect the welded brackets on a piece of thin sheet metal when her hair got caught in the conveyor belt. The employee used her left hand to try to pull herself free when her left hand got caught in the conveyor belt resulting in amputation of the left pinky fingertip.

An employee was moving a shaker panel with an overhead crane. A separate shaker panel had been leaned vertically on a wooden crate along the walkway. As the employee was operating the overhead crane, the vibration caused the vertical shaker panel to fall over and strike the employee, fracturing the employee's left tibia.

Tension built up and caused the roll to swing out from the machine. The employee sustained multiple injuries, which led to hospitalization: internal injuries, possible right arm fracture, three broken right-side ribs, and 6-inch laceration to right abdomen.

An employee struck a wall while parking a stand-up Toyota lift at the end of the shift. The employee's body hit a metal safety suggestion box, fracturing the employee's pelvis. At the time of the incident, the Toyota lift was missing a safety strap that drags on the floor to dissipate static electricity.

An employee removed a worn out sprocket from a Brehm Tube loader machine. The sprocket was repaired and placed back into the machine. The machine was energized and failed to automatically feed. The employee opened the machine while the machine was energized to try to engage the machine by pushing a chain with his left index finger. His finger was caught in the machine resulting in an amputation.
