On November 16, 2021, a truck driver was removing straps from a load of utility poles on a flatbed trailer. When he removed the last strap, the top pole on the load rolled off and struck the left side of his body, crushing his left wrist and the lower part of his left leg. He was hospitalized, and the leg was medically amputated below the knee. He also suffered a brain injury and two fingers were medically amputated.

An employee was disassembling a roughly 55-pound gusset plate because it was assembled incorrectly. As he removed the last pin, the plate started to fall off of the table. He tried to catch the plate, and his left-hand ring finger was caught between the plate and a section of another metal piece. His left ring fingertip was amputated without bone loss.
