On October 20, 2020, a maintenance employee was cleaning a machine pit. The employee was lifting a wooden box and it slipped out of his hands. When he tried to grab the wooden box, he pinched his left middle finger between the wooden box and a metal corner on the machine resulting in amputation of his left middle fingertip.

The employee powered down an air handling unit. While it was slowing down, the employee removed a motor guard when it slipped onto the belt/sheave below, making noise and beginning to burn the belt. As he attempted to move the guard, the belt pulled his left fingers into a pinch point, partially amputating the middle finger and crushing the index finger. The machine was not locked/tagged out at the time.
