An employee was taking out old parts of a 690-volt electrical panel. When the employee removed one of the panel's doors and placed it next to the live side of the panel, an arc flash burned both of the employee's hands. The employee was hospitalized.

Employees were placing front tongue load-bearing metal poles into metal stand boots. A metal pole weight slipped, and an employee's right thumb was caught between the pole base and the boot rim. It was amputated at the first joint. The employee was hospitalized and the thumb was surgically reattached.
