On or about 12/16/15, an employee was opening the gate for a semi truck to leave the area. The semi trailer's rear bumper with the hazmat sign got hooked on the gate. The gate broke loose from the hinges and went flying 10 feet in the air landing on the employee. The employee was hospitalized with fractured ribs, a potential hole in the lung, and a facial laceration.
