An employee was attempting to clear what he believed to be a clogged blower chute from the spin trimmer. As he was shaking the chute to clear it, the chute disconnected from the blower unit. He pulled a rolling ladder to the unit and then attempted to reconnect the chute to the blower. His left hand slipped and entered the motor where the impeller blades struck and amputated the fingertips of his index, middle, and ring fingers.

An employee was working in the extruder area preforming purging activities when the employee became injured by the material compound and sustained burns to the left side of the face and right wrist. The employee was hospitalized.

On August 30, 2017, an employee was cleaning out/blowing down a rotary airlock when the airlock caught and amputated his left middle and index fingers. The airlock was not locked/tagged out at the time of the incident.
