A temporary employee was closing the door on a loaded tractor trailer. A bar was in place in the E-track to keep the product off the door. The employee's finger was caught between the E-track bar and the closing door, resulting in a right ring fingertip amputation as well as broken middle and pinky fingers.

An employee used a stationary horizontal band saw to cut a piece of sheet metal about 15 feet long. When the employee went to move the piece of sheet metal, the end hit an adjacent portable band saw, causing the portable band saw to tip off its stand. The employee walked over to upright the tipped over portable band saw and it completely fell off the stand.
