On September 24, 2021, an employee was shackling the hind leg of a hog and placing it onto a rail. The employee went to hang the hook on the rail when a pusher came into contact with the hook and began sliding it down the rail. To avoid a fall, he put his right hand down in the sprocket and chain. At about 5:20 p.m. on September 26, 2020, an employee was on a table about 2 1/2 feet high, checking for a blockage in a metal chute that deposited sirloin onto an overhead conveyor.
