An employee parked a forklift on a ramp. When the employee got off the forklift to close an overhead door, the forklift started rolling. On May 18, 2020, at approximately 10:15 a.m., an employee was helping another employee cut sheet aluminum and when his left thumb entered the shear at the same time that the other employee stepped onto the lever. The shear then amputated the employee's left thumb tip.

An employee was running meat through a saw. His metal mesh glove came into contact with the sawblade, which pulled his left middle finger into the saw and caused a partial fingertip amputation.

An employee was exiting the cab of a tractor to hook up airlines from the trailer. As he put his right foot on the bottom step, his foot slipped and he fell backwards to the ground onto his left hip. The fall resulted in a fracture to his left hip that required hospitalization.

On May 29, 2018, the injured employee was standing between two automated guided vehicle (AGV) train cars adjusting a crate. Another employee drove alongside the injured employee's AGV in his own AGV. Cargo on the second employee's AGV struck a car on the injured employee's AGV train. This caused the cars to fish tail and the injured employee's left leg was caught between two cars fracturing the fibula.
