On June 27, 2020, an employee was entering a fertilizer bin through the top access when the employee fell off the bin's internally mounted ladder and struck internal structural members and the bottom of the bin. The employee was hospitalized with a fractured right arm, fractured left shoulder, fractured ribs, and cracked vertebrae.

A temporary employee was cleaning a mold while preparing it to be placed in a mold press. The employee stepped down some steps and lost balance. While attempting to catch themselves, the employee's hand went into the machine's conveyor system and was caught between the roller and the mold plate, partially amputating the left ring finger.

An employee was on the ground helping a front-end loader recover a disabled vehicle (semi/tractor). The operator of the front-end loader did not see the employee and dropped the bucket, which struck the employee and caused back and lung injuries.

On or about September 9, 2016, an employee was unrolling scrap film from one roll and placing it onto another roll when the edge of the roll slipped off the roller bearings. The employee and a coworker attempted to lift the roll approximately 1/16" back onto the roller bearings, but the roll shifted, pinching and amputating the meaty portion of the left middle finger between the roll and the cradle.
