An employee drove a forklift into the aerosol room with a pallet of product on the forks. She drove down the back driveway to the aisle of racking she needed to turn into. The forklift drove through salt brine that was on the floor and slid into a concrete bollard at the end of that aisle. The employee's left foot got caught between the bollard and the forklift. The employee was hospitalized with four broken toes and a laceration to the left foot.

On or about June 25, 2015, an employee working at the Army Corp of Engineers Ensley Engineering Yard located at 2915 Riverport Rd in Memphis, Tennessee, was injured and hospitalized. The employee was operating a forklift to move an air conditioning unit up a ramp when it began to fall backwards. The employee was injured when he was caught between the forklift and the wall of the dry dock, resulting in a severe laceration to his left wrist.
