An employee was helping to install a forging hammer sow block. He was standing near the hammer and guiding a forklift operator who was manipulating the suspended block. The employee was directing the forklift backward to realign the block and had his hand on top of timber that was connected to the load strap. As the block was coming out, it contacted the bottom of the hammer and caused the timber to rotate. The employee's hand was crushed between the timber and the forklift tine.

Employee was driving a standard roll-off truck for picking up containers of spent material from various metalworking companies in the area. He was on-site to pick up a metal box type container from a customer. The employee hooked the truck's wire rope cable and winch up to the box and engaged the winch briefly. The box was moved close to the bed of the truck and raised a few inches off the ground. The winch was disengaged. He then placed a tarp over the box. As the employee was placing the
