An employee was drilling holes in a part using a Bridgeport mill. The part moved in the vice and the employee reached in to re-secure the part while the machine was running. A burr or the drill bit snagged his glove pulling his thumb into the drill bit and causing a partial amputation of the left thumb.

Employees were moving a heat exchanger tube bundle with a hydraulic crane. The crane had set the tube bundle down on a crawler and the employee had disconnected the synthetic sling from the bundle and used hand signals to the crane operator to rise the main line. The sling caught the tube bundle and rolled off onto the employee.
