On or about 7:30 a.m. on October 26, 2015, an employee was preparing to change a powered industrial truck battery. Due to the end stops on the multi-level battery rack being bent out of place, the battery was positioned too far into the rack for the large suction cup on the automatic battery machine to reach it and pull the battery out. The employee used a strap to try to pull the battery forward and in doing so, his left pinky finger was pinched between the strap and a guardrail in the area,
