While securing tank caps, an employee fell from the side of a paint truck when their pipe wrench came loose. They landed on the asphalt surface of a parking lot. The employee suffered a collapsed left lung, contusions to the hands and face, neck pain, and was hospitalized.

An employee had just lowered a bundle of scaffold frame to the ground with a crane. He released the chains from the bundle and was walking away when the scaffold frame fell onto the back of his right arm and his right little finger. The fingertip was amputated.

An employee was leaving a patient care room when she fell forward, landing on her arms. She suffered a fracture to the right arm between the shoulder and the elbow.

An employee was in a manhole, tending a ladder while a pipe plug was being deflated. The pressure inside the pipe caused the plug to pop out of the pipe and strike the access ladder inside the manhole. The ladder in turn struck the employee in the throat, causing a neck contusion.
