An employee was on a 7-foot scaffold pumping concrete when the hose became plugged and started whipping around. The hose hit the employee and he fell 7 feet from the scaffold. He was hospitalized with fractures to his pelvis.

Two employees were removing sheets of plywood from old shelving when a piece about 3' by 6' slipped. The injured employee's right pinky fingertip was caught against the plywood and the skin tore off (no bone damage) within the employee's glove. The employee was hospitalized.
