On May 19, 2015, at approximately 11:00am, an employee was hospitalized after being struck in the head by a 3 inch rubber air hose fitting. The employee was operating a drill rig at the time of the incident. A hose under 300 pounds pressure, broke at the fitting (swivel came off the threads) allowing the hose to whip and strike the employee in the back of the head. The employee fell 5' from a platform resulting in a fractured skull.
