An employee was taking apart a round spool of wire, which weighed 1,100 pounds. The spool was standing up on the round edges and was then knocked flat on the ground. The employee's foot was caught by one of the round ends of the spool, causing a compound fracture to the left ankle that required hospitalization.

Two employees were working on a scaffolding system that toppled over injuring both employees on May 5, 2016, at approximately 0930 hours. One employee fell directly onto a small structure near the scaffold and suffered a broken pelvis, shoulder, ribs, and wrist on his right side. The employee was hospitalized for treatment of his injuries. The second employee fell directly on the bare ground and suffered a small laceration on his leg. The second employee was treated and released.
