A door on the platform swung open and the employee fell backward to the floor, landing on the back left side of his body. He suffered six broken ribs on the left side and was hospitalized.

Two employees were working on a scaffolding system that toppled over injuring both employees on May 5, 2016, at approximately 0930 hours. One employee fell directly onto a small structure near the scaffold and suffered a broken pelvis, shoulder, ribs, and wrist on his right side. The employee was hospitalized for treatment of his injuries. The second employee fell directly on the bare ground and suffered a small laceration on his leg. The second employee was treated and released.
