An employee was preparing to reinstall a box chain that had broken. When he tried to rotate the drive belt manually (in order to straighten the chain and thus find its end), the chain shifted and the drive belt rotated in the opposite direction. His right ring finger was pulled in and trapped between the sheave and the V-belt. The fingertip was amputated. | |||||

Employees were making up float shoes on the first joint of a casing job. The pipe came loose, swung, and caught one of the employees between the manual back-up tong and the back-up snub line. The employee was hospitalized and treated for injuries to the chest and colon area. |
