An employee was pushing a newly cut section of beef down a conveyor belt. A hook, used to reach and hold the piece in place, did not release when the employee pushed the piece down the conveyor. The piece of beef then pulled the employee off balance. The employee's right hand slipped into the sawblade, which amputated the index finger.

He was hospitalized.
