An employee was screwing in drywall while standing on approximately the middle rung of a 6-foot A-frame ladder. The employee leaned out to the left to reach his work when the ladder tipped over sideways. He grabbed a metal stud with his left hand in an attempt to stop his fall and his foot got caught in the ladder during the fall. He sustained lacerations from the metal stud and a fractured hip caused by contact with the ground. The employee was hospitalized.

An employee was on a roof to attach the roof ladder to the peak of the roof. The ladder slipped while he was trying to attach it to the roof. The employee lost balance and fell approximately 15 feet to the ground below suffering injury to his right side and shoulder area and damage to his mouth and teeth.

A painter was walking up the indoor stairs. He fell from the second floor landing down to the first floor breaking his collar bone, five ribs and punctured a lung. It is new construction so there is no railing on the stairs.

An employee was walking on stilts (raised up about 3 feet), installing batt insulation in the ceiling (about 8-feet high) with a manual staple hammer. The stilts caught a piece of plastic insulation bundle wrapping, causing the employee to lose his balance on the stilts. He tipped forward on the stilts, striking the floor with his knees. His coworkers offered to help him back up, but they couldn't move him. He was hospitalized with a fractured spine.
