| An employee was performing maintenance on a setting tool used in hydraulic fracturing. He opened a vise and turned the tool 180 degrees to make it easier to service. | An employee was operating a dump truck and another employee was outside the truck attempting to shut the tailgate. The tailgate was stuck open and the employee was pulling on the tailgate trying to get it unstuck.
