An employee was attaching a C-hook to a smaller scale hook attached to an overhead crane. The C-hook was resting on top of a coil. The smaller scale hook was not fitting into the C-hook and the C-hook slid off of the coil, striking the employee's left shin and causing a laceration.

Four employee were pouring a 320" copper stock. A hose failed which caused the ram of the casting unit to drop during the pouring process. The falling of this ram, which was controlled by the caster, allowed molten copper to come in contact with steam inside the pit that is filled with water. This caused a violent reaction/explosion. All four employees received burns to their arms, body, and face when they were sprayed with molten copper.

On Saturday, August 11, 2018, four employees were unloading a barge containing structural steel. One employee was operating an overhead crane remotely when the cable jumped off the hoist. The injured employee went on the crane to see if there was anything he could do to get it back on track. While he was trying to get the cable back on track, tension developed in the cable, severing the ring, middle, and index fingers of his left hand and the index and middle fingers of his right hand.
