An employee was operating a CNC bandsaw with an automatic material loading shuttle and clamping mechanism. The wood that the machine was shuttling into the clamping area was tipping, and the employee reached over to stabilize it for the clamp. The clamp closed on the employee's right index finger and amputated the fingertip.

The employee left the cooling room to report to his supervisor that he was feeling unwell. While his supervisor was talking to him, the employee collapsed in the supervisor's office and was unresponsive. The employee was hospitalized for heat stress.

An employee was using his hands to push and slide a paper roll onto the forks of a forklift when the operator inadvertently hit a button in the cab and the forks started to raise, causing the roll to move towards the employee. The employee began to run/jump out of the way when the roll struck the employee and pinched his leg against a second roll that was sitting on the floor behind him. He suffered a left fibula fracture.
