An employee was operating a stand up counterbalance in the case pick area when she collided with pallets in the racking. The corner of the upper Chep pallet penetrated the operator compartment, striking the employee in the upper hip area and fracturing the pelvis.

Employees were descending a water tower in suspended scaffolding. About 30 to 40 feet above the ground, one of the motors locked up and the scaffold stopped. One of the employees detached his rope lanyard from his harness and attempted to slide down another rope lanyard. The employee fell off the rope hitting the ground and fracturing both legs and suffering severe rope burns on this hands.

On 2/21/15, an employee's fingertip was amputated. The finger was caught between two bricks on a convert belt and was torn off. The employee was taken to Louisiana Medical Center and Heart Hospital.
