An employee was preparing to climb a tree. To set his climbing line, he threw a throw line and throwball in the tree. The throwball got stuck in the tree and he pulled on the throw line to dislodge it when it came toward him and struck him under his right eye. The employee suffered a fractured orbital bone that required hospitalization.

On 6/13/2016, at 1:30 a.m., an employee was cleaning a ventilation hood when his foot was caught in the lower rung of a ladder he was descending. He fell approximately 6 to 8 feet, fracturing the 2nd and 3rd metatarsals in his foot and requiring surgery.
