On October 29, 2015, at approximately 4:00pm, a maintenance technician suffered an amputation while troubleshooting an air cylinder on an evaporator foam fixture in the Tooling Department. The maintenance tech placed his left hand on top of the cylinder hole while looking at the right side of the cylinder.
