A brace surrounding the photo eye on the stretch wrapper machine was loose, causing the photo eye to be misaligned. This caused the chain to stop on the conveyor. An employee knelt down to adjust the brace. His hand was on the chain guard on both sides of the brace when the photo eye became aligned and the chain started up again. The chain caught his right middle finger causing a partial amputation to the distal phalanx.

On April 23, 2020, at approximately 8:25 p.m., an employee was adjusting the position of a conveyor belt. He grabbed the belt with both hands in order to pull the belt over. As he was pulling the belt over, the fingers on his left hand were pinched between the belt and roller, amputating his left ring fingertip and fracturing his left middle finger.

An employee was clearing a jam in a panel/film and tensioner wheel. The employee raided the tensioner wheel set and began pushing the panel/film out of the wheels. The thumb of the his left glove became caught in the tensioner wheel assembly, pulling his hand through the top and bottom wheels. Once his hand travelled through, he was able to free it. His left thumb fractured.

An employee was using a metal shear to shear an 8' x 16" wide aluminum scrap roofing material to 4" x 4" test strips when his hand went under the guard and the stops came down and crushed both of his index fingertips. His left index finger required a 5 mm amputation from the tip end of the bone. His right index finger was debrided.

An employee and a coworker were operating a saw with dual palm buttons. The employee was on the left side holding a 2 x 4 wooden board to be cut toward the back stop on the machine. The coworker was on the right side and actuated the palm buttons while the employee's left thumb was in the point of operation. When the blade came up, it amputated approximately 1/4" of the employee's left thumb tip. The saw was unguarded at the time.
